By accessing the website, you are agreeing to be bound by these terms and conditions, all applicable laws and regulations, and agree that you are responsible for compliance with any applicable local laws. If you do not agree with any of these terms, please leave the website as you are prohibited from using or accessing this website. 

The materials contained in this website are protected by applicable copyright and trademark law. 

INFORMATION

  • All patients are required to provide contact details; including address, telephone and email in order to secure an appointment.
  • All clients will complete a Medical history form at their first appointment, this is necessary to form the consultation, prescribing and treatment planning process.
  • All information will be treated as confidential and protected in accordance with Data Protection legislation (GDPR).
  • Patient information will not be shared with third parties without written permissions and you will not receive unsolicited information from us.
  • You may choose to remove yourself from our mailing list at any time, by unsubscribing.

APPOINTMENTS

  • You will be sent appointment reminders before your appointment via email. These reminders are a courtesy, please do not rely upon them.
  • At Chronic Pain & Injection Clinic, we ask for 48 hours’ notice for cancellations for appointments.
  • If you fail to attend or cancel late, then you may incur a fee of £50 (except) for face-to-face consultation.
  • Please do NOT attend the clinic for an appointment if you are unwell. If you are unsure, please call and discuss with our friendly staff who will be happy to help. Many treatments are contraindicated if you are unwell.
  • Routine review appointments (telephone / face-to-face) may be offered after the treatment as a courtesy.
  • New patients are seen for consultation and assessment only; we do not offer treatment on the first visit. This is to allow adequate cooling off period and help our clients to make an educated decision.

CHILDREN

  • We do not treat children or young adults under the age of 18.
  • Please do not bring children to the clinic unless they are old enough to be left unsupervised. If you do attend the clinic with a child, we reserve the right to cancel or reschedule your appointment.
  • Children will not be allowed to accompany you into the treatment rooms.

PAYMENT

  • For face-to-face consultation we ask for the £150 payment upfront to secure your appointment. The invoice will be sent via email and the payment is to be made via bank transfer. 
  • You will be advised of the full costs of any treatment plan proposed and agreed, including that of any maintenance treatment, before any treatment is undertaken.
  • Payment is taken, in full at the time of treatment.
  • The clinic accepts all major debit and credit cards except American Express.
  • A discretionary deposit will be taken for some treatments and in some circumstances, this will be payable via bank transfer. Bank details will be provided via email.

REFUNDS

Fees charged for treatment are for the delivery of a treatment and the accompanying service, which is inclusive of;

  • Consultation & Assessment (Face to face consultation fees is only refunded up to 48 hours prior to the appointment)
  • Provision of information and advice
  • Safe treatment with evidence-based products
  • Follow up appointments and aftercare advice and support as appropriate

Whilst we undertake to provide excellent service; factual, honest and ethical advice, safe, expert treatment in experienced hands and only the best products, we cannot guarantee your results and cannot offer refunds if the results achieved fail to meet your expectations.Products are non-returnable/refundable.

COMPLAINTS

If you have a complaint please inform us as soon as possible.
An appointment will be made for you to be seen by Dr Baba.
A copy of our Complaints Policy is available online or on request.

CCTV

We employ CCTV on the exterior and public areas of the premises in order to aid the security of our premises, staff and patients. CCTV cameras are not installed in our treatment rooms. 

All CCTV footage is held for a maximum of 14 days. Please note in certain circumstances we may need to disclose CCTV footage for legal reasons, a Subject Access Request may be made for the footage we hold.
